CENTRE D’ÉVALUATION GÉRONTOLOGIQUE
Like all Geriatrics Assessment Centres (CEG), it is intended for elderly people who are dependent or have lost their autonomy living at home. CEG are located at the interface of city medicine and hospitals. It is usually the treating physician who sends the elderly person into an EMC. The consultations conducted by the CEG aim at caring for the senior «fragile» person, through an interdisciplinary and multidimensional approach; development of preventive and screening measures (fall, malnutrition, etc.); the standardized evaluation with a proposal for actions always associated with medico-social monitoring; and keeping home with any follow-up, also insured at home, to prevent, for example, unplanned emergency admissions.
